CareAllies Health Coaches and Care Coordinators
Expert Help for Managing Your Health
You have a new partner for staying healthy and feeling your best. Ohio State
is now offering health coaching and care coordination programs through
CareAllies (www.mycareallies.com ,
password is "buckeyes"), a leading third-party provider of health and wellness
programs. Health Coaches and Care Coordinators can help you:
- Develop strategies for achieving a wide range of health goals, through
diet, exercise, weight management, and prevention.
- Act on any identified health risks.
- Better manage chronic conditions.
Health Coaches: Anyone who completes a Personal
Health Assessment (PHA) can take advantage of the health coaching benefit,
which is provided for you at no additional cost. A CareAllies Health
CoachSM is a certified health educator who will be available to help you set
health improvement goals and provide ongoing guidance and expertise tailored
to your needs. Your Health Coach may also recommend a Lifestyle Management
program for health concerns related to tobacco cessation, weight management,
or stress reduction. Based on certain risk scores in your PHA data (and
whether you consented to releasing this information to the health plan),
you may be contacted directly by a Health Coach to work with you on making
changes that could help minimize the effects of any health concerns.
Care Coordinators: If you’re living with a chronic
condition such as diabetes, asthma, chronic obstructive pulmonary disease,
or heart disease, you understand the importance, and sometimes the difficulty,
of managing these conditions. Ohio State provides a care coordination benefit
through CareAllies for you at no additional cost to supplement the care
being provided by your doctor. Our care coordination program is called
CareAllies Smart Steps,SM and Care Coordinators are
Smart Steps nurses who are trained to help you recognize and anticipate symptoms,
comply with your care plan, and get the support that you need between physician
visits.

| Who’s eligible? |
All benefit-eligible faculty and staff and their enrolled adult
dependents who have completed a PHA and have authorized the release
of their PHA data to the health plan. |
All faculty and staff and their dependents who are enrolled in an
Ohio State medical plan and are identified as being eligible for one
of the programs based on PHA results or analysis of medical and prescription
drug claims. |
| How do I sign up? |
You may be contacted by a Health Coach based on certain health risks
that are identified in your PHA, or you may contact a Health Coach
directly to address health concerns. |
You may be referred or invited to the program based on a medical
diagnosis, risk factors, medical history, PHA results, or medical and
prescription claims. |
| What services does this program provide? |
- Specific health information on topics like nutrition, exercise,
high blood pressure, high cholesterol, prevention, and more.
- Coaching assistance to help you make changes that can minimize
the effects of any health issues.
- Referrals to resources and other programs from which you may benefit.
- Lifestyle Management programs in specific areas such as tobacco
cessation, weight loss, and stress reduction.

- Help in taking control and minimizing day-to-day health effects
of asthma, diabetes, heart disease, chronic obstructive pulmonary
disease.
- Assistance with becoming more proactive in managing your
condition.
- Development of an action plan with a Smart Steps nurse
and your own physician’s input.

- Know how to access important routine care and appropriate exams.
- Develop healthier habits and condition management strategies.
- Understand and comply with your doctor’s recommendations.
