2008 Faculty and Staff Incentive Program

Live Well and Earn Money for It

Join Your Plan for Health’s 2008 Faculty and Staff Incentive Program today and begin earning points for a healthier you. The revamped Incentive Program offers many of the same health-related activities plus a few new ones! And now your payout will be included in your paycheck at the end of the quarter in which you earn it, rather than at year’s end. Here’s how to make healthy living pay off for you.

All benefit-eligible faculty and staff who completed their Personal Health Assessment (PHA)* can begin tracking points for the Incentive Program by visiting www.yp4hpoints.com.

*PHAs taken in 2007 qualify; however, be sure to take the PHA again this year (and annually) to continue qualifying for this program and the premium discount. Please note: If you are completing the PHA after January 1, 2008, access to the Incentive Program may be delayed for up to three weeks due to eligibility processing.

Program Details:

  • New for 2008, the PHA is no longer a points-eligible activity since taking the PHA entitles you to an annual medical plan premium deduction of $120. However, you must complete the PHA to be eligible to participate in the Incentive Program.
  • PHAs taken in 2007 qualify, but be sure to take the PHA again this year (and annually) to continue qualifying for future programs including premium discounts.
  • Earn up to 125 points by participating in health-related activities by December 31, 2008.
  • Activities have been assigned a point value depending on their potential impact on your overall health and the personal effort required to participate. Take part in only one activity to receive some points or several activities to reach the maximum 125 points.
  • Points will be awarded based on participation, not an individual’s health status or outcomes. Participation in the Incentive Program is voluntary.

Eligibility:

  • Annual completion of the PHA.
  • Benefit-eligible faculty and staff: Medical plan enrollment is not required. Affiliated groups: Medical plan enrollment is required. Graduate Associates (GAs): Enrollment in a university faculty/staff medical plan is required.
  • Must meet eligibility guidelines when activities are completed to receive the payout.
  • Must be an employee at the time of payout to receive incentive dollars.
  • Dependents of faculty and staff who are benefit-eligible employees can also participate.

Here’s How to Get Started:

  • First-Time Participants:
    Once you link to the points-tracking web site, go to the “First Time Here?” box to create a personal profile using your Employee ID number.
  • Participants With Existing Accounts:
    If you created a points-tracking profile for the 2007 Incentive Program, access your existing account by entering your username and password in the “Already Have a Profile?” box. If you can’t remember your password, click on the “Forgot your password?” link.

Once you log on, click on the “Points-Tracking Program” link on the left to confidentially track incentive points online. Matria Healthcare, a third-party provider, administers the Incentive Program site. By participating, you’ll be rewarded for understanding the value of good health, engaging in health and wellness activities, and helping to foster a more productive university workforce.

Your Rewards

Accumulated points will translate into money (1 point = $1) paid directly to you, less applicable payroll taxes. Incentive payouts will be made quarterly to those who earned the maximum points during that quarter. For example, if you’ve earned and tracked the 125 points for the Incentive Program by March 31, you will receive your one-time payout of $125 in April. Not only will you earn a few extra dollars, but you will also be making healthy choices by engaging in health-related behaviors while helping to control rising health care costs.

Examples:

125 points earned = $125
$125 – 30% tax = $87*
75 points earned = $75
$75 – 30% tax = $52*
50 points earned = $50
$50 – 30% tax = $35*
25 points earned = $25
$25 – 30% tax = $17.50*

Health-related workshops Attend a workshop or view a web cast. Workshop must be offered by network provider, hospital, or OSU Faculty and Staff Wellness Program. Four workshops on different topics are allowed. 10 points each

Nutrition counseling Visit with a licensed registered dietician. Participants can receive a free nutrition consultation through the Faculty and Staff Wellness Program. Participants enrolled in a university- sponsored medical plan have additional options that offer three free visits through the nutritional counseling benefit.

  • Participants enrolled in a university medical plan can receive three visits with a licensed, registered dietician, no referral required. Visit osumhcs.com/

Please note: Many of the 2008 Faculty and Staff Incentive Program activities are available at no cost to you while some have fees for service, which may be covered by your insurance. Participation in the Incentive Program does not guarantee insurance payment. Please check your plan documents or contact your insurance provider to verify coverage.
